Med-File is a personal medical portfolio to provide the information you need to participate in your medical care. The portfolio enables you to keep all your medical information in an organized indexed booklet. The medical portfolio contains forms that you fill out and keep inside the organizer. The portfolio is divided into various tab sections (see Med-File Table of Contents). Each section contains forms that are a color coded to match the tab. The portfolio layout allows for easy retrieval and filing.
